Clinical Pharmacy Specialist - Job Overview

Key Responsibilities
- Engage in groundbreaking pharmaceutical care services through direct interactions with patients and providers
- Utilize cutting-edge virtual care technology to manage chronic disease state medications
- Extract comprehensive medical and/or medication histories to ensure data-driven, safe, and effective care
- Innovate in medication reconciliation at ambulatory care clinic visits
- Deploy advanced physical assessment techniques and triage patients as needed
- Document patient interactions and interventions in electronic medical records with precision
- Design and implement therapeutic plans utilizing the most effective, least toxic, and economical medications
- Leverage approved procedures to modify, renew, refill, or discontinue medication therapy
- Order and interpret labs and tests to optimize medication therapy outcomes
- Report and manage adverse drug events and medication allergies
- Ensure compliance with all regulatory authorities while practicing innovative pharmacy
- Provide cutting-edge drug information and education on interactions, side effects, and more
- Evaluate and enhance medication adherence during counseling sessions
- Demonstrate leadership in setting priorities and working collaboratively across departments
- Serve as a mentor and preceptor to pharmacy students and residents
Minimum Qualifications
- Graduate of an accredited college/university of pharmacy (PharmD or B.S. with equivalent experience)
- Completion of ASHP accredited PGY-1 Pharmacy Residency
- Valid Texas Board of Pharmacy license
- Certification in Basic Life Support / First Aid / CPR
- Board-Certified Pharmacotherapy Specialist (BCPS) or Board-Certified Ambulatory Care Pharmacist (BCACP)
- 2+ years of community pharmacy experience
- 2+ years of medication adherence counseling experience
- Specialized experience in HIV/AIDs, Geriatrics, and/or Pediatrics
- Completion of ASHP accredited PGY-2 Pharmacy Residency in relevant fields
- Certification in immunization delivery or smoking cessation
- Proficiency with Windows, Microsoft Word & Excel
- Ability to manage multiple digital projects and prioritize effectively
- Excellent communication and customer service skills
